This form must be completed and approved by the Catering Exceptions Committee when any group intends to receive assistance with food preparation. Failure to comply with this policy could result in the loss of this privilege.

Describe completely all food and beverages to be served/prepared at your event as well as how they will be prepared. Vague or incomplete descriptions will delay approval. You will need to provide a copy of the recipe to UNI Dining Services.

Follow-up Instructions

  • Once completed by the sponsoring organization, this form will be submitted electronically to the Catering Exceptions Committee and you will receive a copy of the request.
  • Once you receive your email copy of the completed form, you will need to obtain the remaining approval signatures and turn your form into the Catering General Manager, Redeker Center 010. The completed form should be turned in as far in advance as possible, preferably 8 (eight) weeks in advance of the event so that there is plenty of time to organize and plan the event with UNI Dining Services. The specific menu and recipes (include number of servings needed) for each menu item in addition to any other documentation requested by UNI Dining Services should be approved with this form.
  • If approved, the Catering Exceptions Committee will notify the sponsoring organization/department when approval has been granted.

Terms & Conditions

  • This form must be completed a minimum of 8 (eight) weeks prior to the event.
  • Students should follow all instructions given by UNI Dining Services staff including appropriate dress and sanitation practices as required by the Iowa Food Code prior to and during food preparation and clean-up.
  • A provided “Waiver of Liability” form must be signed by all students who assist in the preparation of the food in DOR operated facilities. This form will be provided by UNI Catering at the time of the event.
  • The number of students needed for food preparation will be determined by UNI Dining Services staff. If the recommended number of students are not available for food preparation, the event may be canceled. Students should stay until UNI Dining Services staff approves the work is complete.
  • All costs associated with the event are the responsibility of the sponsoring organization/department. This includes menu ingredients, use of kitchen and serving equipment, professional staff time and any other requirements deemed necessary by UNI Dining Services.
  • Future permission to engage in similar activities is contingent upon compliance with these terms and conditions.
